October 4th will be the FMS Flu Clinic. If you are interested in your child receiving the Flu Vaccine, the paperwork will be sent home on Monday or Tuesday of next week to be filled out.
No child will be forced to receive the Flu Vaccine.
Your child will have to have the form filled out(front and back) and returned on or by Thursday, September 29th. If your child does not have this turned in by 9/29, the vaccine will not be administered on 10/4.
